OfficeMax Feedback Survey
- Complete a short survey related to a OfficeMax purchase and receive a coupon
All customers who complete the OfficeMax Feedback survey will receive the coupon via email within 48 hours and the coupon will come in the form of a $5 off coupon that can be used at any OfficeMax location. In order to provide feedback to OfficeMax customers will need the following from their in store receipt:
- OfficeMax survey code
- Date of purchase
- Time of purchase
The OfficeMax Feedback survey code will be 11 digits long and the date/time of purchase can be found at the top of the receipt under the OfficeMax store address. The survey will take about 5 minutes to complete and customers MUST provide a valid email address in order to receive the promotional survey coupon.
